Do you wish you had a helpful progress note template?  One that would ensure that you include in each session note everything that insurance plans and oversight agencies are looking for in an audit?

I designed this progress note template to do just that: To make sure your session notes meet health plan documentation requirements including Medicare, as well as those required by licensing boards, ethics codes, and state laws.

Unlike many progress note templates, it doesn’t overly rely on checkboxes – something insurance plans dislike. With too many checkboxes you can end up with notes that are too superficial, vague, or repetitive, leading the plan to deny payment.

This progress note template includes checkboxes (to help you write the note quickly) but also includes space for narrative comments, to record essential details about topics the client discussed, their symptoms, your interventions, and progress made. It has all the essential items for telehealth sessions, including the location of the client, that you have obtained telehealth consent, and that the client is deemed appropriate for telehealth, as is required by many state laws.

Narrative sections expand to provide space for longer comments in each section, if needed.

This template will help all therapists to find a good balance between spending too much time writing notes and not writing enough details to capture the need for treatment.

The note template is provided in three versions as part of this package:

  • Fillable PDF format (the template can’t be altered)
  • Word format (you can alter the template)
  • Pages format (you can alter the template)

If you have an Electronic Health Record, you may be able to upload and/or customize this template, or it can be used to design your own template.  It can also be used if you handwrite your notes, or write notes kept in files on your computer.
