This must-have list gives you the names and addresses of more than 50 insurance plans and Employee Assistance Programs that operate in California. Best of all, the addresses are in mailing label format, so you can print the list onto mailing labels, if desired, saving time and typing. Apply only to the companies you are interested in. Or apply to all of them, and sit back and see which ones respond, and choose between them.
