Since it is impossible to keep up with the ever-changing addresses related to health plan credentialing, we know you will find some of these addresses to be out-of-date.  But we hope this will give you a place to start.

The labels include the names and addresses of more than 50 insurance plans and Employee Assistance Programs that operate in California, and are easily printable onto mailing labels to save you time and typing. Print using Avery 8160 labels or any mailing label sheet that has 30 labels per sheet.  Some health plans are represented with more than one address if it was unclear which was the best for credentialing materials; we recommend you send your request to join to all addresses.
