Where Does it Hurt? The Importance of Documenting Impairment

Your progress notes should always answer the question, “how is the client’s life being impaired by his/her mental health symptoms?” This may sound obvious, but when a client is high functioning, it isn’t always easy.
Documenting impairment is important — and essential to arguing “medical necessity” for treatment, which insurance plans use for coverage decisions. It can also help a client win disability or worker’s compensation cases.
So — when documenting impairment, what areas should you think about?
